RN Case Manager/Discharge Planner

Job Summary and Essential Functions:
Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements relative to his or her age and specific needs, and provide the care needed as described in departmental policies and procedures. Works with minimal supervision. Must have excellent communication skills both verbal and written to address individuals of varying educational levels. Normal office environment with some travel to other facilities.
Daily communication with all members of the case management team, patients, families, nursing staff, physicians, managers and external organizations to coordinate patient care, resolve problems meet patients' post discharge needs.
Job Requirements
Education: Registered Nurse; Bachelor's Degree preferred.
Required Experience: 2-3 years of recent experience in a health care setting with responsibilities including planning, coordination, and delivery of case management services such as education, psychosocial support, discharge planning and utilization management.
Licensure: Current Indiana Nursing License; Certification in Case Management preferred.

This position requires a thorough knowledge of State and Federal regulations regarding medical necessity for admission and discharge screens. Knowledge of community resources, discharge planning and referral to nursing homes, extended care and rehabilitation facilities. Must have the technical skills necessary to demonstrate progressive care programs to patients and/or their families. Frequent interactive associations are inherent in this position. The ability to retrieve, communicates, and presents data and information verbally and in writing is required. Strong interpersonal skills and the ability to communicate effectively with patients, families, nursing staff, physicians, and community resources is required.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
